    By submitting this form, you acknowledge that you understand the Booth Sale Etiquette and Guidelines written in our Volunteer Cookie Manual. This form is used to assist Council and Service Unit Product Program Managers with ensuring that booth rules are being followed by volunteers.

  • The following informatin is optional. Please complete the questions to the best of your ability. 

  • Are there 2 registered adults OR is it a Caregiver/Daughter booth?
  • At 2 adults and 1 girls must be present at all times during a Cookie Booth. When girls in attendance are not supervised by their own caregiver, at least one of the adults present must be registered and background checked. 

    If this is a Caregiver & Girl Scout booth (also known as a Caregiver Cookie Booth); the caregiver must be registered and background checked. 

  • Is the Caregiver registered and background checked?
  • Reminder that every booth sale must have two volunteers present at the booth sale. One of these volunteers must be background-checked. 

  • Are the Girl Scouts in Girl Scout attire or wearing their uniform (sash/vest) or have their insignia tab pinned?
  • Are the Girl Scouts handling the payments?
  • Are the Girl Scouts eating or drinking at the table? Food and/or beverages should NOT be on the same table used for selling Girl Scout Cookies.
