Prior to your visit please fill out this form to the best of your ability. I love designing your look to match your face shape, personality, and lifestyle. To make sure we are hitting those points I like to revisit these questions about twice a year, so if it has been 6 months since you have filled this out please do so before your appointment. Likewise if you have never filled out this form please do so.
Because I like to make all my services custom and unique to you, I require all new clients to schedule a 15 minute consultation before scheduling any type of chemical service with me. This gives us the opportunity to discuss your desires, expectations and if your hair can handle the service. After your consultation, if we decide to move on, we will book your first chemical appointment together. Once you are done completing this form you will be directed to my booking site where you can schedule your consultatation or haircut service.

All chemical services require special at home after care. If you are not using my product recommendations, and following after care instructions I can not guarantee your service.*

Currently our cancellation policy is to allow us 24 hours notice that you are unable to make the appointment. This will hopefully allow us time to schedule someone else into your spot. Consistent last minute cancelations and reschedules can result in a 30% cancelation/reschedule fee and/or not being able to reschedule your appointment.*
