This AGREEMENT is between Helium L.L.C. (Salon) And The Under Signed (Bridal Client) On the Day, Month and year of Date*
This will GUARANTEE your appointment times on the wedding date when the following have been completed.- This agreement must be completed in full and Signed.- A deposit of 50% of All services to be rendered as chosen by you on the Bridal Agreement, must be paid in full.- Client Must Have Valid Credit card on file, with an Expiration date EXCEEDING wedding date.- all of the above must be completed 30 days prior to wedding date. 4. ATTENDANCE & CANCELLATION Policy:- Late Arrivals: In order for each service to be preformed properly, if bridal client or another guest in the party who is RECEIVING services under this agreement, is not present or available to RECEIVE services until 15 minutes or more after his or her AGREED service time, Bride client agrees to pay a late fee of $20.00 per late arrival.5. NO-Show:- If the bridal client or ANOTHER GUEST IN THE PARTY WHO IS RECEIVING SERVICES UNDER THIS AGREEMENT, IS NOT PRESENT OR AVAILABLE TO RECEIVE SERVICES until 30 or more minutes of his or her agreed service time, the individual will be considered a No-show. resulting in paying 100% of the price, as a No-show fee, of the services that were to be provided to said guest. 6. Cancellations & appointment Changes:-all cancellations and appointment changes must be made 30 days before the wedding date.-including adding services, removing service, changing times, or altering any appointments.-In the event of a cancellation or appointment change made less than 30 days before the wedding date, bridal client agrees to pay cancellation/APPOINTMENT change fee equal to the prices of all services previously agreed to, regardless of whether or not preformed on the wedding date.7. Gratuity: -a 15% gratuity will be added to the charge for all services preformed by the salon on the wedding day.8. Trials:- A Trial appointment is for THE purpose of determining the style, product, length of time needed, or any other considerations necessary for the wedding date services. Though not required it is RECOMMENDED, there is a charge for all trial services rendered.-The bridal client is responsible for scheduling the trial appointment no later than a week before the wedding and no early than 30 days before the wedding. -The salon has no duty to arrange or schedule the trial appointment.9. Pre-wedding services: the salon RECOMMENDS some pre-wedding services to make the wedding day go smoother and more flawless. These services are however not required just RECOMMENDED. All services are to be schedule by the bridal Client themselves. 10.Payment Policy:- Bridal client must provide a valid credit card with an expiration date EXCEEDING wedding date. In the name of the bridal client. -This card will be charged for ALL services no later than 30 days before the date of service.-Any changes made after 30 days MUST be paid in full.-Gratuity will be charged to the card on file on the final service date-Bridal client agrees Helium salon may charge the card on file for all bridal client services, bridal guest services, late arrival fees, no-show fees, and/or cancellation/ appointment change fees, or other fees set forth by this agreement. 11.Liability: All brushes, tools & makeup products are SANITIZED in according to regulation & salon standards. Any allergies and or skin CONDITIONS must be reported by bridal client or bridal guest prior to RECEIVING services. Bridal client agrees to release , INDEMNIFY and hold harmless helium salon from any and all liability ARISING from any skin complication due to allergic reaction or other complication that arises from styling or makeup application.12.Assignment: -the rights and obligations provided by this AGREEMENT are SPECIFIC to the salon and bridal client. Neither the salon nor bridal client shall assign, sublet, or transfer interest in this AGREEMENT with out written CONSENT of the other.13.Waiver:No waiver of any PROVISION of this AGREEMENT shall be effective unless it is in writing, signed by the party against whom it is asserted, and any such written waiver shall only be applicable to the specific instance to which it is related and shall not be deemed to be CONTINUING or further waiver. No waiver of any provision of this AGREEMENT, whether express or implied, shall be construed as a CONTINUING or further waiver of the same or and other provision of this AGREEMENT.14. When Scheduling all bridal services we will rendered time slots availbe for services you have selected. From that point we will build a mock itinerary and send it to you. In that Intinerary we will have time slots that will need filled with a name. Remember you will be responsible for sharing the schedule with your guests.
