Photo Release
I hereby give to MOVE/ Open Bible Churches, its successors in interest and assigns, the unrestricted right and permission to copyright and use, re-use, publish and republish photographic portraits or pictures of me in which I may be included intact or in part, composite or distorted in character or form, without restriction as to changes or transformations in a conjunction with my own or a fictitious name, or reproduction hereof in color or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade or any other purpose whatsoever.
I also permit the use of any public display of material in a connection therewith, either in printed or digital form. I hereby relinquish any right I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it is applied.
I hereby release, discharge and hold harmless MOVE/ Open Bible Churches, its successor or assigns, its legal representatives and all persons functioning under its permission or authority, or those form whom they are functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion or us in composite form whether intentional or otherwise, they may occur or be produced by the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasions of privacy.
I hereby affirm I am over the age of majority and have the right to contract in my own name. I have read the above authorization, release and agreement, prior to its execution. I fully understand the contents thereof. This agreement shall be binding on my heirs and me: legal representatives and assigns.
Waiver of liability
The undersigned does hereby consent to take part in the noted short-term overseas construction project organized by, MOVE and Global Missions, a ministry of Open Bible Churches, a religions, non-profit corporation. I have been informed that a foreign country is very different from the United States. I have been apprised that there are risks politically, culturally, safety wise, and health wise and that problems in any or all of these areas may arise during the overseas mission assignment. In addition, I have been apprised that the threat of terrorism agains Americans increases on foreign soil. I am also aware that the threat of terrorism in air travel both from America and from foreign airports. I also understand that this trip will involve missions activities, construction activities, and mingling directly with people of a foreign country. I am willing to assume whatever risk may arise from this overseas mission trip. I hereby agree to assume all responsibility for my own health and safety while overseas and traveling to and from the field. I agree to assume all expenses occasioned by any injury or loss and do wholly release MOVE and Open Bible Churches/Global Missions from any responsibility or liability, and waive any claims or causes of action against it, its agents, employees, and volunteer assistants which may arise on account of any loss, injury or expense occasioned by any sort of accident or other circumstances involving any foreign or travel risk and agree to hold harmless Open Bible Churches and the Global Missions in the event any such claims should arise; and
I also declare that I am not an employee, agent or independent contractor of MOVE or the Global Missions and that my services on the project are entirely voluntary and a donation on my part and no compensation whatsoever shall be paid to me.
I have read the above waiver of liability, and I understand its contents and import and agree to its provisions. I hereby declare that this release is executed without reliance upon any statement or representation of MOVE, Open Bible Churches or Global Missions or any agent, employee, or volunteer thereof and that the execution is made voluntarily and knowingly.
