    • Introduction - Plan Managed Options - ConneXtions 
    • Plan Managed Options is one of the services that ConneXtions delivers as a registered NDIS Service Provider.

      This means that participants can access Plan Managed Options regardless of any other services delivered by ConneXtions.

      This is a Service Agreement for provision of Plan Management Services for your National Disability Insurance Approved Plan (NDIS Plan). To engage the services of ConneXtions – Plan Managed Options, you need to complete this service agreement and upon acceptance of this Service Agreement, agree to provide a copy of your NDIS plan.

    • This Service Agreement is for Plan Management Services as part of the National Disability Insurance Scheme and is made between:

    • When does this Service Agreement commence? 
    • This Service Agreement will take effect from the date listed on the NDIS Plan, and/or dates detailed in within this Service Agreement.

      Please note: Connextions Plan Managed Options can only process invoices within the plan period. NDIS may terminate an NDIS plan early, or extend its duration. All changes impact processing of your claims for payment. So, if you are issued with a new plan, or your plan ends early, you must advise ConneXtions immediately.

    • When does this Service Agreement end? 
    • Once completed, signed and dated by the participant and/or nominee, this Service Agreement will remain in place until ConneXtions is notified in writing that the agreement is to end.

      Should either party wish to end this Service Agreement they must give one month (28) days notice in writing. Unless otherwise negotiated by both parties.

      During the notice period, both parties will work towards a smooth transition of Plan Management services to ensure continuity of supports to the participant. 

      If either party seriously breaches this Service Agreement the requirements of notice will be waived. 

    • Responsibilities of ConneXtions - Plan Managed Options 
    • ConneXtions – Plan Managed Options agrees to:

      •  Provide a service agreement outlining our terms of business to all participants and/or nominees
      • Create service bookings in PRODA for your plan-managed budgets 
      • Consult participants, and/or nominees upon request, regarding funding expenditure decisions
      • Process claims through the NDIS portal in line with set plan dates
      • Pay provider invoices on behalf of the participant
      • Process reimbursement claims for expenses
      • Track expenditure against plan-managed budgets
      • Provide monthly statements
      • Assist with purchasing equipment, where appropriate
      • Communicate openly, honestly and in a timely manner
      • Treat participants / nominees with courtesy and respect
      • Listen to the participant/ nominees feedback and resolve problems quickly
      • Protect the participant’s privacy and confidential information
      • Welcome customer feedback information regarding both compliments and complaints.
      • Give the participant and/or nominee the required notice if the provider needs to end the service agreement 
      • Continue to provide Plan Management services until you notify us of your intent to cease services.
    • Responsibilities of Participant / Nominated Representative  
    • I the NDIS Participant / Nominee agree to:

      •  Take all reasonable steps to provide information, as requested, to ConneXtions – Plan Managed Options, in a timely manner
      • Treat ConneXtions – Plan Managed Options staff with courtesy and respect
      • Discuss concerns about services with ConneXtions Plan Managed Options staff
      • Advise ConneXtions – Plan Managed Options immediately if my NDIS Plan is suspended or replaced by a new NDIS Plan, or terminated by NDIS
      • Take responsibility for Service Agreements I willingly enter into which are outside my NDIS Plan funding allocations.
      • To ensure continuity of service, provide a copy of your NDIS Plan when it is issued, where you require ConneXtions – Plan Managed Options to continue to provide plan management services.
      • Agree to provider share plan consent to ConneXtions on myplace NDIS portal
      • Advise ConneXtions – Plan Managed Options first if you no longer require us to act as your plan management provider.
      • Understand that my needs may change over time, however changes in my service provision will be limited to within my plan funding budget caps
      • Accept responsibility for my actions and choices even though this may involve some level of risk.
      • Consent that the NDIS Commission, its delegates, and/or external auditors can review my file. Please note that you can rescind this consent at any time by contacting ConneXtions - Plan Managed Options.
    • Paying your Providers - How your Service Providers get paid? 
    • Once service bookings have been set in the NDIA Portal (PRODA), ConneXtions – Plan Managed Options will claim from the NDIS, funding up to the amounts specified in the support category and budgets, as approved in the Participant’s current NDIS plan.

      After supports are provided, service provider invoices can be forwarded to the participant / nominee and/cc to accounts@connextions.org.au for payment. The invoice is then claimed from the NDIS and the service provider is paid.

      We aim to claim and pay providers within 3-5days, unless we are advised otherwise.

      Please note: If there are any issues with the services you have received and/or the invoices please advise us immediately so that we do not pay the provider until the issue is resolved.

      Careview Advantage

      ConneXtions - Plan Managed Options, your Plan Manager has a tool called Careview Advantage to help you with your NDIS journey.

      Careview Advantage is an App for your phone which lets you view your Plan Managed NDIS funding information in real time. 

      It allows you to view your funding budget, manage invoices and check if your funding is being utilised in a timely manner. 

      Gives you the option to approve or decline individual invoices.

       

    • Reimbursements  
    • This may be relevant if you have paid for a service or an item which relates to your NDIS plan and you require reimbursement.

      To ensure compliance with the NDIA, you will now need to show a tax invoice/receipt as proof of purchase for Connextions - Plan Managed Options to reimburse the funds to you.

      The easiest way to do this is to send the tax invoice/receipt to accounts@connextions.org.au along with your bank details (bank details must be provided to  ConneXtions Plan Managed Option in writing). Once Connextions - Plan Managed Options receives this, we can process the payment to your account.

       

    • Changes to this Service Agreement 
    • If changes to the supports or their delivery are required, the parties agree to discuss and review the Service Agreement. The parties agree that any changes to this Service Agreement will be in writing, signed and dated by the parties. 

    • Feedback, Complaints and Disputes 
    • If the participant/nominee wishes to provide feedback, or is not satisfied with the provision of supports and wishes to make a complaint, the participant/nominee can:

      If the participant is not satisfied or does not wish to speak with ConneXtions, the participant/nominee can contact the NDIA by calling 1800 800 110, visiting one of their offices in person, or visiting www.ndis.gov.au for further information. 

    • Goods and Services Tax (GST) 
    • For the purposes of GST legislation, the parties confirm:

      • A supply of supports under this Service Agreement is a supply of one or more of the reasonable and necessary supports specified in the statement included, under subsection 33(2) of the National Disability Insurance Scheme Act 2013 (NDIS Act), in the Participant’s NDIS plan currently in effect under section 37 of the NDIS Act;
      • ConneXtions – Plan Managed Options will pay GST as per specified in National Disability Insurance Scheme Act 2013 (NDIS Act)
      • Participants / nominees will receive invoices for services that may or may not state that GST is payable for that service. Differences occur due to some services being exempt for GST and some service providers not being registered for GST.
    • Consent to Share Information 
    • Please Note: ConneXtions Plan Managed Options will not provide information to any party without consent to share. This protects your privacy and confidentiality.

      Giving additional parties consent to access your personal information is purely optional and will not affect our services. You have the ability to add or remove people to your Consent to Share list in the future.

      Consent to Share enables a participant to give signed consent for ConneXtions - Plan Managed Option employees to discuss personal and plan information over the phone or via email with additional parties such as Secondary Contacts (your parent, a co-parent / guardians, grandparents, close friends etc), Support Coordinators or others.

      Only you (Participant), your Guardian or Authorised Representative may give consent to share. The information we share can include your current budget amounts, information about recent invoices and providers you have engaged, client details and representative contact details. You can tell us what you want to share.

      Giving additional parties consent to access your personal information is purely optional and will not affect our services. You have the ability to add or remove people to your Consent to Share list in the future.

    • Provider Service Agreements - Why we need them?

      A great way to make sure there is adequate funding to cover your provider invoices from your NDIS budget is to send Connextions Plan Managed Options a signed Service Agreement. Service agreements allow us to allocate a set amount of funding from a particular budget for the support you receive.

    • Plan Management - Financial Administration Supports

      As a Registered Plan Management Provider ConneXtions claims directly from the NDIS for services provided from Category 14 – CB Improved Life Choices. Fees are determined by the NDIS, and updated regularly.


      These support items allow participants to engage a Plan Manager to manage and monitor budgets over the course of the plan, manage NDIS claims and pay providers for delivered service, maintain records and produce regular (at least monthly) statements showing the financial position of the plan.

    • Disclaimer 
    • ConneXtions – Plan Managed Options acknowledges the constant and rapid changes occurring across the NDIS landscape. Our services are delivered in good faith, and our understandings are based on NDIS instructions and provider information. All information received from NDIS and each party to this Service Agreement is considered true and accurate.

      Outside of financial intermediary services, all information provided by ConneXtions – Plan Managed Options, shall be considered general in nature. ConneXtions bears no responsibility and will not be held liable for any failure of, or delay in performance of this Service Agreement for any period, that such failure or delay is:

      1. Beyond the reasonable control of the party
      2. Materially affects the performance of any of its obligations under this agreement, and
      3. Not able to have reasonably foreseen or provided for (eg: Government Acts Prohibiting or impeding the party from performing its respective obligations under Service Agreement or contract).
      4. ConneXtions – Plan Managed Options Service Agreement does not negate or diminish the statutory guarantees regarding the supply of services to the Participant/nominated representative, received under the Australian Consumer Law (Competitive and Consumer Act 2010 Schedule2).

      ConneXtions – Plan Managed Options takes in good faith, the information provided by the Participant/Nominated Representative as true and accurate.

      All claims presented to ConneXtions – Plan Managed Options, are genuine in nature, and accurately reflect provision of goods and services to the Participant. These are in line with NDIS Guidelines, and aligned with the National Disability Insurance Scheme Act 2013 (“The Act”).

    • In signing this Service Agreement, you appoint ConneXtions – Plan Managed Options as your nominated NDIS Plan Manager. This Service Agreement will remain valid until such time as you instruct us to cease as your NDIS Plan Management provider.
