Recovery Pathways Submission Form

I confirm that I understand and agree to each of the following:

• The Local Dug Recovery Evidence Base project is part of an IAF funded initiative in collaboration with the University of Derby.
• I am happy to participate in this project and have done so voluntarily
• Submissions are accepted where the work is completed by a person with lived experience of alcohol or other substance use dependence.
• The submissions will be used for inclusion on a website which will naturally be publicly available worldwide and is being curated by a researcher and a web designer.
• One photo and text describing what the photo represents and how it has helped your recovery per submission is required.
• You can submit more than one entry.
• Submission does not guarantee inclusion but we would hope that entries are used on the website except in exceptional circumstances.
• The reason for non-inclusion will not always be informed but we will try our best to make contact to discuss.
• Non-inclusion is not a reflection of the quality or creativity of the submission.
• All submissions may be used for other publications such as academic journal articles, conferences, exhibitions or other media as part of the dissemination of the project.

• I understand I can submit my entry and request that my name and any other specified details are not included publicly if I wish.
• I understand I can ask questions both prior to and after I have submitted my artwork

• I understand I can withdraw my consent for my submission to be used by emailing d.patton@derby.ac.uk
