Food Box Request Form
We are so pleased to be able to assist families in need in the Dothan area with a box of food once a week on Thursdays. To be added to our list, simply fill out the form below before 12pm (noon) on Thursday. (THIS FORM MUST ONLY BE SUBMITTED ON THE THURSDAY YOU ARE REQUESTING A BOX. ANY FORM SUBMITTED ON A DIFFERENT DAY, OR ON THURSDAY BUT AFTER NOON, WILL NOT BE CONSIDERED.) Once submitted, you'll receive a confirmation of your submission. Then, between noon and 1:30pm, you'll receive a text from Dothan 1st either confirming your food box and how to pick it up, or you'll receive a text letting you know that we didn't have enough food boxes that day.
