The Outdoor Ice Oval Society of Prince George collects and uses personal information only as reasonably necessary for its programs and activities.  The Society does not share this information with others.

I hereby make application for registration for membership in the Outdoor Ice Oval Society of Prince George and do hereby agree to obey the Constitution, Bylaws, Rules and Regulations of the OIOS of PG.  

I am fully aware that the Outdoor Ice Oval is unsupervised and that outdoor skating has many risks including, but not limited to, those associated with ice and weather conditions. I agree to skate at my own risk. 

In consideration of you accepting this application, I hereby for myself and my heirs, executors and administrators, waive and release any and all rights and claims for damage I may have against the Outdoor Ice Oval Society and their respective directors, officers, agents, representatives or successors for injuries suffered by me at the Outdoor Ice Oval in its entirety.
