F.L.A.G.S. CAMP APPLICATION

Please note - the FLAGS Application is a 3-step process.

1. Fill out and submit the following application.

2. Forward an online recommendation form to your pastor or youth leader for him to fill out on your behalf. (The link to the form is found here as well as toward the end of this application.)

3. Submit the video portion of the application as instructed within this application.

You will then receive a phone call from a TMBC Staff Member regarding your acceptance into the TMBC FLAGS Program.

After you have received confirmation of your acceptance, you will be emailed a link to a separate form that will allow you to pay your registration fee or the full camp fee.

Sunscreen Permission
I give my permission for the staff at Treasure Mountain Bible Camp to assist with applying or to apply sunscreen to my child's exposed skin including the face, tops of ears, and bare shoulders, arms, legs and feet 30 minutes before outdoor activities. It is my responsibility to provide sunscreen with a minimum SPF of 15. I understand I must provide the sunscreen in its original container labeled with my child’s name and within the noted expiration date. Sunscreen will not be applied to any broken skin or if a skin reaction has been observed. Any skin reaction observed by staff will be reported promptly to the parent/guardian.

I hereby give permission for my child to attend and participate in all of Treasure Mountain Bible Camp’s activities on/off the camp property. I hereby absolve Treasure Mountain Bible Camp from liability to me or my child because of an injury received while attending camp at Treasure Mountain Bible Camp. In case of any accident or serious illness, I hereby authorize Treasure Mountain Bible Camp to call upon a physician of their choice and to follow their instructions. If emergency treatment or hospitalization is required, I request Treasure Mountain Bible Camp to notify me. I also understand, if my child is sent home for any reason, there will be no refund. I also understand that my camper’s picture or testimony may be used in the promotion of the camp.

VIDEO APPLICATION
Please take a video of yourself answering the following prompt (no more than a 5 minute long video) and then submit the video using the instructions below:

F.L.A.G.S. is a leadership camp. Please submit a short video introducing yourself. Then give an explanation of you what you think leadership is all about. This is purposely broad, and you can use examples, quotes, and Bible verses, but try to keep the answer YOUR own answer.

To submit your video, send it to the camp office via email to office.tmbc@gmail.com or via text to (970) 930-7859.

We highly recommend taking a picture of these instructions so that you can complete this task correctly.
