**The income/expense amounts on this form will be used to calculate each parent's Court-Ordered Child Support obligation. **
THIS FORM IS NOT THE COURT'S FINANCIAL AFFIDAVIT FORM. YOU WILL BE REQUIRED TO FILL OUT THE FINANCIAL AFFIDAVIT AFTER MEDIATION IS COMPLETED.
CHILD SUPPORT WILL BE INCLUDED IN THE SETTLEMENT AGREEMENT AND IS CALCULATED BASED ON THE MONTHLY NET INCOME AMOUNTS SHOWN ON THIS FORM (OR ON ANY UPDATED VERSION).
THE COURT WILL REFERENCE THE INCOME AMOUNTS ON YOUR FINANCIAL AFFIDAVIT TO VERIFY THAT CHILD SUPPORT WAS CALCULATED CORRECTLY.
(1) Please enter your First name, Last name, Email address, and Current date.
(2) Please fill in ALL applicable INCOME $ amounts and all applicable PAYROLL-EXPENSE $ amounts.
- Enter the $ Amount for all applicable Income Categories
- Enter the applicable Pay-Period for that Income $ Amount (e.g., Weekly, Bi-Weekly, Annually, etc.)
- Enter the $ Amount for all applicable Payroll Expense Categories
- Enter the applicable Pay-Period for that Payroll Expense $ Amount (e.g., Weekly, Bi-Weekly, Annually, etc.).
Some people use average amounts (e.g., average of prior months or years) when income or expense amounts are inconsistent (e.g., wage, bonus, or commission payments) or if self-employed. However, the manner to calculate such income is on you alone, as the court's financial affidavit will require accurate information, sworn under penalty of perjury.
This form will automatically calculate your Monthly Net Income based on the $ amounts entered and the applicable pay periods.
Once you click the Submit Button, you (and Eric) will receive an email with a .pdf copy of the completed form attached. Eric will upload the submitted form to the shared Dropbox folder for their review.
If you ever need to make changes, please follow the Edit link in the email you will receive after submitting the form. You can also save your progress and resume the form later by clicking the Save Button and following the instructions.
