1. Type of Change. What level of change is your program introducing?

Technology Enhancement - A technology upgrade or other simple change that does not require new processes or user skills
Business Change. A change in business processes and technology with limited organization or culture changes (e.g., ERP implementation)
Transformational Change. A complete shift in culture, organization structure, behaviors, strategy, norms, processes, and technology.
2. Leadership. Is your executive leadership team taking an active role as an advocate for the change?

Yes - Leadership has taken ownership of this change and are driving it in their organizations
Somewhat - Leaders have been communicated to but there is still uncertainty if they understand what the change means to them and their organizations
No - Leaders are hands off, expecting the program to deliver a solution without active sponsorship
3. Culture. Will this program require a change in business culture to be successful?

Yes - This change will require significant culture change as we shift to a new organization structure and operating model
Somewhat - There will be some change in culture required to deal with the new ways of working as we move to more integrated processes and technology
No - There is no culture change anticipated with this program as there is low impact to processes and skills
4. Org Structure. Will this program require a change in organization roles, responsibilities or structure?

Yes - The organization will require changes, including potential changes in job descriptions, roles and responsibilities, new organization and reporting structures, and possible elimination of some jobs
Somewhat - There will be changes to organization roles and responsibilities, but no major org structure changes
No - There will not be any organization changes in roles, responsibilities, or structure
5. Skills. What level of skill change will be needed to adopt new processes or concepts, and possibly, new ways of executing transactions?

Major - the change is major and users will need to learn completely new processes or competencies
Medium - There are changes in business processes and technology which will require a training program, but the fundamental jobs will not change
Minor - users will need some new skills but limited training required
6. Motivation. Is the user base motivated to adopt requisite changes or is there resistance you can foresee?

Motivated - Users are motivated to adopt changes and are looking forward to the implementation
Unaware - Users are unaware of any requisite changes or don't understand the impact of the change to them
Resistant - Users will be resistant to change as they do not see the benefit in the program or have had previous negative experiences

9. Internal OCM Capabilities. Do you have an OCM department established within your organization to handle change efforts?

Yes - We currently have dedicated resources that help manage and facilitate IT & Business change programs
Somewhat - We have a few individuals who know how to do OCM but have not managed or facilitated a program of this magnitude
No - We have no significant OCM expertise in the organization
10. Benefits. Are the key business benefits expected from this program enabled by a change in processes, skills, or organization?
