DELIVERY DRIVERS ONLY: I, the submitter of this electronic form, hereby certifies that on or about Deliveries 25, 2022, agree to use my personal vehicle and fuel as a volunteer of Greater Lighthouse Community Outreach, Inc dba The Blessing Corner for the 2022 Christmas Day Extravaganza and other Community Events. I will operate said vehicle and further certify that I am a lawfully licensed driver and hold an active comprehensive and collision policy. I will be operating as required by the state and federal laws. I agree that should anything happen as a volunteer or in association with the event listed above, I will NOT hold Greater Lighthouse Community Outreach, Inc dba The Blessing Corner and or its partners or affiliates responsible or liable for any cause that may occur in my role today. By completing this electronic form, I hereby agree my acknowledgement and agreement.*
