Online Registration Acknowledgement - I/We certify that all provided information is correct and true. If any of the submitted information is found to be incorrect or untrue, this will deem this registration form invalid and will have to be re-submitted online for approval.*

Permission to Use Name and Likeness for Publicity - I/We hereby grant the Community Youth Council (CYC) the right and permission to use and publish any photographic and/or likeness of player for the purpose of promoting CYC, use on website and waive any rights to compensation related to use of player image and/or name.*

Concussion Protocol Policy and Return to Play Protocol - I/We understand, agree and will adhere to the CYC Concussion Protocol Policy and Return to Play (RTP) Protocol. If any CYC official observes any player to exhibit concussion related symptoms, or hears a player complain of concussion symptoms, that player will be removed from the game. Player must be cleared by a licensed health care provider and provide written clearance from such health care provider as well as a completed/signed Return To Play form prior to next game played.*

Code of Conduct Policy - I/We understand, agree and will adhere to the CYC Code of Conduct Policy. The Code of Conduct policy covers the behavior of the player, coach, parent and spectator involved with CYC and must be upheld before, during and after the contest. I/We also will ensure that our son has a full understanding of said policy and will abide fully.*

Covid-19 Policy - I/We, understand that this is a recreational league and that accept the responsibility and liability in the event of an accident or injury. Moreover, I/we acknowledge the contagious nature of COVID-19, that I/we may be exposed to COVID-19 as a result of participation in the league, and that such exposure or infection may result in personal injury, illness, permanent disability, and death. By signing this form, I/we release the League Organizers, Participants, CYC, and the facilities I/we participate in from any and all liability. As a condition to participate in the said league, I/we agree to sign this document, free of duress and agree to all terms and conditions.*
