  • Welcome to the VITA program at Clover!

    We're happy to serve you and look forward to meeting with you soon.  

    See below to select the best location and time for your in-person appointment. 

    You MUST bring all required documents (listed below) at the time of the appointment to be served. 

    Tax information can also be dropped off to the front desk at the Main Campus or at the Columbia Parc location (1401 St. Denis St/Parking lot entrance) between the hours of 8:00 am and 3:30pm. We will notify you when they're complete so the required reveiw can be scheduled before submission.

  • SCHEDULE YOUR APPOINTMENT

    Appointments Can Only Be Made For One Location (1/17-4/17)
  • Please bring copies of the following documents

    • IRS Form (F13614-Consent Form) If you can't print this form and complete it in advance of your appointment, please arrive 15 minutes early to do so. 
    • Photo IDs, & Social Security cards for you, spouse, & dependents listed on return  
    • W-2s, 1099Rs, 1099-G, 1099-MISCs, etc.
    • Interest & Dividend forms (1099-DIV, 1099-INT)
    • Any other related documentation you have received (1099, 1098-T, etc.)
    • Total paid for daycare (provider’s name, tax ID#, Address, & Phone number)
    • Identity Theft IPIN letter from IRS (if applicable)
    • Health Insurance Form 1095A from the Marketplace (Obama Care)
    • Bank routing & account numbers for direct deposit (Voided check or bank statement)
    • Uber/Lyft drivers MUST provide official mileage and documentation of expenses from your account
