Group Event Request Form

Thank you for considering having McKamey Animal Center participate in your event! We are so grateful for each and every request, but as you can imagine, we cannot commit to every request and still meet the needs of all of the animals in our facility and throughout the community. Please complete the form below providing us with more details about what you have in mind for your event so that we can determine if we have staff available to serve you. Please note that submitting a request does not guarantee our participation or commitment -- we will send you an email confirmation or send our regrets within 5 business days. Also, requests should be submitted at least 30 days prior to your requested event date, as fulfilling last minute requests may not be possible. IMPORTANT:

For safety reasons, we can accommodate no more than 20 visitors at a time. Also, if anyone in your group will be under the age of 18, there must be at least 1 adult, ENGAGED chaperone for every 9 minors (chaperones must remain with the group at all times and must support MAC staff in ensuring the safety of each child). We will require submission of the names and ages of group members, along with signed liability waivers, before arrival -- anyone not on the final list of attendees and/or without a waiver will not be able to participate. Disclaimers

A MAC staff member will reach out within 5 business days to let you know whether your request can be honored. Please note the following disclaimers and requirements:
By signing below, you understand and agree that if your request is approved the following provisions will apply:
- No more than 20 total visitors can be accommodated at MAC at a time. Any group that arrives with more than 20 visitors may not be permitted onsite.
- Any minors must be accomodated by adult, ENGAGED chaperones (1 chaperone for every 9 minors). Chaperones must remain present during the entirety of the visist and must actively participate in ensuring the safety and welfare of the minors. Children may not just be "dropped off," nor may chaperones disengage from the group and its activities.
- Scheduled events may be altered or canceled by MAC last minute if natural disasters, communicable disease outbreaks, or other unforeseen events occur.
- All approved events must have final confirmation of event details provided at least 48 hours before; if there has been no communication with requestor 48 hours before the event is scheduled to begin the event will be presumed canceled.
- All attendees wil be required to sign liablity waivers in advance of their arrival at MAC.
- Groups must submit a final list of attendees prior to arrival at MAC. Anyone not named on that attendee list may not be permitted on site.
