Reservation Request Form - Saturday 5/17

• Please ensure all fields below are filled out. Double check that your contact information is entered correctly, as we primarily will communicate via email.
• Once we receive your completed form we will contact you within 3-5 business days.
• You will receive a response from waterworksvt@gmail.com. Please add this email address to your contacts. If you're concerned you might have missed a response, check your spam folder!
• We will be taking reservations for groups up to 14 guests. For groups of 15 or more, please submit an event inquiry for event room pricing and availability.
• Dinner reservations for parties up to 8 guests will have a 2-hour dining time
• Dinner reservations for parties of 9 guests or more will have a 2.5 hour dining time.

Cancellation within 7 days of your reservation will result in a $100 charge to your credit card. *Your card will not be charged if you cancel at least 7 days prior to your reservation.

Party size changes within 24 hours of your reservation are subject to a fee of $20/person for the number of guests that the party size decreases by.

Please note, the card you enter to confirm your reservation will not be used to charge the balance of the dinner, unless that card is brought in on the evening of your reservation.
