Read Before Signing
As legal guardian of the participant mentioned below, I consent to his/her participating in the Basketball Training offered by Raffi Boyadjian and his team . I am aware that participation has some inherent risks and injury can occur. I hereby agree to, assume all risk, for allowing my child to participate in Basketball Training and I agree to hold harmless and indemnify Raffi Boyadjian and his team from any and all liability, for any and all damages, suffered by my child while under his instruction and supervision. This release of liability and wavier has been read thoroughly and is agreed to in content and intent, and I am willingly signing this form. : This form must be signed by your parent and/or legal guardian.

I hereby acknowledge that I am the parent/legal guardian of the "Minor Participant" (Minor Child) above who is under the age of eighteen. In consideration for the benefits to be derived from My Minor Child’s participation in the basketball training, I, on behalf of myself and my Minor Child, hereby acknowledge the following: (1) I am aware that all basketball activities involve risk; (2) On behalf of myself and my Minor Child, I further understand and acknowledge that the risks of basketball training activities may result in injury.

Because of the danger of participating in the above activity, I, both on my own behalf and on behalf of my Minor Child, acknowledge and understand the importance of following the rules and regulations established by Drive to the hoop with Raffi. My minor child hereby agrees to obey such rules, regulations, and instructions.

I further acknowledge that my Minor Child is in good physical condition and do not know of any condition or reason that my Minor Child should not participate in basketball training.

ON BEHALF OF MYSELF AND MY MINOR CHILD, I HEREBY RECOGNIZE AND ASSUME ALL THE RISKS ASSOCIATED WITH MY MINOR CHILD PARTICIPATING IN BASKETBALL TRAINING AND RELATED ACTIVITIES. I AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS DRIVE TO THE HOOP WITH RAFFI,RAFFI BOYADJIAN, AND HIS TEAM AGENT REPRESENTATIVES, AND VOLUNTEERS FROM ANY AND ALL OBLIGATIONS, LIABILITIES, CLAIMS, DEMANDS, COSTS, AND EXPENSES, INCLUDING ATTORNEY’S FEES, OR DEMANDS OF ANY KIND OF NATURE WHATSOEVER WHICH MAY ARISE OR IN CONNECTION WITH MY MINOR CHILD’S PARTICIPATION IN ANY ACTIVITIES RELATED TO BASKETBALL TRAINING, INCLUDING INJURIES SUSTAINED AS A RESULT OF THE SOLE, JOINT, OR CONCURRENT NEGLIGENCE, NEGLIGENCE PER SE, STATUTORY FAULT OR STRICT LIABILITY OF RAFFI BOYADJIAN AND HIS TEAM.

The terms hereof serve as a release and assumption of risk for myself and on behalf of my Minor Child, as well as our respective heirs, estates, executors, administrators, and assignees.
In signing this agreement, I acknowledge and represent that I have read and understand it; that I am the parent or legal guardian of the named child; that I sign it voluntarily and for full and adequate consideration, fully intending to bind myself and the named child by the same; and that I am at least eighteen (18) years of age and fully competent.
