Instructions:
1. Take a blood glucose measurement before feeding your pet and record the value and time in the worksheet.
2. Feed your pet the typical amount.
3. Administer insulin as directed by your veterinarian.
4. Obtain blood glucose readings as directed by your veterinarian. It is typical to get a reading every 2-4 hours but this may differ from your veterinarian’s recommendation for your pet.
5. Fill in the time you took the reading and the glucose measurement.
6. Submit to your veterinarian for evaluation. Never make insulin adjustments without consulting your veterinarian.
