A $250 commitment deposit (first 10% of your project cost) is due with this form in order to reserve your space on OP.

If someone else will be paying the deposit on your behalf, please reach out to Danielle Miles (danielle.miles@campusoutreach.org) to receive an alternate form.
Upon completion of this form, you will automatically receive an email with information on how to sign up for Managed Missions - the platform that the project will use to process payments and track finances. After you sign up, you will recieve instructions via email within 24 hours (Mon-Fri) on how to begin raising support or make further payments towards the remainder of your total project cost.
