Cranes, Rigging and Suspended Loads : Lifts, rigging hardware, load path, swing radius. (Load drift into walkway, shackle misused, unplanned movement)
Mobile Equipment and Traffic : Forklifts, trucks, earthmovers, spotters, pedestrian/vehicle separation. (Near back-over, blind-spot contact, unsecured load shift, no spotter)
Energy Isolation (LOTO / Zero Energy State) : Electrical, mechanical, hydraulic, pneumatic, gravity, kinetic. (unexpected start-up, stored energy not released, missing lock)
Electrical (Shock/Arc Flash) : Energized work, testing, temp power, cords/GFCI (Live panel exposure, arc boundary breached, damaged cord in wet area)
Confined Space / Hazardous Atmosphere : Permit/Non-permit spaces; O2 deficiency; toxic/flammable atmospheres; engulfment (Monitor alarm, ventilation off, entrant without attendant)
Excavation, Trenching and Ground Disturbance : Excavations, utility locates, shoring/shielding, spoils, access/egress. (Wall slough, locate mismatch, ladder more than 25 ft. apart)
Hot Work / Fire and Explosion : Welding/cutting/grinding; open flames; fuel/solvent handling; combustible materials. (Sparks to combustibles, missing fire watch, containers left unsealed.)
Process Safety / Line Opening / Chemical Release : Line breaks, start‑ups/shut‑downs, pressure testing, reliefs, incompatible chems. (Residual product spray, hose whip, over‑press test, relief directed at walk area.)
