    CONTACT UPDATE & ANNUAL DUES PAYMENT FORM

  • The Sons and Daughters of Montana Pioneers Society are developing simplified and cost-effective ways to communicate with our members.
    We are requesting all Members to please update their Membership Contact Information.
    This will ensure that we have accurate information on file so all Members may receive current newsletters, membership updates, and registration information for upcoming Conventions.
    Annual Dues and Lifetime Upgrades are included. A check payment along with the updated information form, is our only option at this present time. We are working towards online services for your convenience in the future.

    Review and make a copy of the form for your records.

    Answering a couple single choice questions will help you fill out this printable form. 

    If you are submitting for self and other members, such as a child or sibling, please select the Multiple Members option, and follow the prompts for each Member.

    Make sure you Review-Print your copy before you select Submit to SDMP for your records.

    Have another Member to Update?

    Submit to SDMP, once selected, will take you to the Thank You Page with the ablitity to Fill Again, for the next Member you wish to update. Some information, such as addresses will be prefilled, so make sure the necessary changes are made.

    This can be done as many times as needed for every Member you would like to update.

  • INSTRUCTIONS BEFORE SUBMITTING PAYMENT TO SDMP

    Make sure you Review-Print 2 copies before you select Submit to SDMP.  One for your records, and the other to mail along with your check payment to the address listed at the bottom of the printed form.

    Upon receipt of your payment you will be issued a SDMP Annual Membership number that will be included on all future forms and Convention Registration.  

  • MULTIPLE MEMBERSHIP PAYMENTS

     

    1. Make sure you Review-Print a copy before you select Submit to SDMP.  This copy is for your records making it easier to recap the Multiple Members in which you are submitting a payment. With every Submit to SDMP, of each member added, I recieve an email with their current updated information.
    2. Submit to SDMP, once selected, will take you to the Thank You Page with the ablitity to Fill Again, for the next Member you wish to update. Some information, such as addresses will be prefilled, so make sure the necessary changes are made. This can be done as many times as needed for every Member you would like to update.
    3. After your final Member Submission at the bottom of the Thank You page is the Multiple Member Recap Form to fill out. 
    4. Review/Print 1 copy of the Multiple Member Recap Form, press Submit to SDMP, and then mail to the address listed at the top of the form with your check payment.
    5. Upon receipt of your payment all members listed on the form will be issued a SDMP Annual Membership number that will be included on all future forms and Convention Registration.
