Please Review the Following Terms and Conditions:

• Households must be within Schaumburg Park District boundaries to be eligible to receive a printed program guide.

• Opt-in lists will be pulled each season to fulfill seasonal guide printing (Schaumburg Park District reserves the right to determine and/or change the list pull schedule). Submissions sent after opt-in lists have been generated will be fulfilled the following season. Please submit only one form per household.

• The program guide is accessible anytime online at parkfun.com.
