PLEASE keep in mind that there are MANY factors that can affect the outcome of your hair goals. I want to make your dreams a reality! the best and only way to make that happens is to disclose ANYTHING that can affect the outcome. Please detail any chemical, color services or any other information regarding your hair history in the last 2-3 YEARS.
General Information and Policies
This is a general consultation form that was created so I would have all neccessary information needed to help achieve your hair goals. We will go over in detail this consultation form during your scheduled appointment once I recieve this form back and your appointment is approved. I will reach out to you before your scheduled appointment if I have any further questions.
Cancellation Policy: I understand that if I need to cancel or reschedule my appointment I need to do so with a minimum of 24 hours notice. If I do not cancel my appointment within the requested time frame, I understand a fee representing $25 for each service missed will be added to my profile and must be paid before I book another appointment.*

I will suggest an alternative product to meet my needs and the value of the product I am returning will be applied towards the new product.
