2023 Joint Spring Seminar Facility Form

This form is designed for program directors to register their students. Each student will receive confirmation and instructions for accessing the sessions.

ASCLS-WA will pay for registration for students in the Washington-based programs. The program director/contact person in other states will be invoiced for payment for their students. That invoice will come from PayPal. If you wish to include the Contact person in the billing, please complete the fields below and check the box to include the contact person. This invoice is payable with a credit card. Please comment if a separate emailed invoice is required rather than that sent from PayPal.
