Troop/Service Unit Donation Form
- This form is used if:
  - The company is making the check directly payable to GSCI for any amount and require a official receipt for tax purposes. For an official receipt for tax purposes, the donation should be made payable to Girl Scouts of Central Indiana and mailed directly to 7201 Girl Scout Ln, Indianapolis, IN 46214.
  - The donation or in-kind donation is valued at $250 or more.
- For an in-kind donation please include the value of the donation in the form.
- Please mark the appropriate box indicating whether the gift is a cash donation or an in-kind donation.
- This form should be submitted only when confirmation is received that you were awarded money from either an individual or another organization such as Kohls.
- Allow 4-6 weeks after we receive this form for a payment to the Troop or Service Unit Bank Account to be issued.
- If the troop is receiving the funds directly and an official receipt is not required, this form does not need to be completed and returned to council.
