Troop/Service Unit Donation Form
- This form is only used if the company is making the check directly payable to GSCI and/or the donation or in-kind donation is $250 or more. For an official receipt for tax purposes, the donation should be mailed directly to Girl Scouts of Central Indiana.
- For an in-kind donation please include the value of the donation in the form.
- Please mark the appropriate box indicating whether the gift is a cash donation or an in-kind donation.
- This form should be submitted only when confirmation is received that you were awarded money from either an individual or another organization such as Kohls.
- Allow 4-6 weeks after we receive this form for a payment to the Troop or Service Unit Bank Account to be issued.
- If the troop is receiving the funds directly and an official receipt is not required, this form does not need to be completed and returned to council.
