    Membership Code of Conduct
  • This form is to establish a safe environment and to create a learning experience when attending H.E.A.L. activities and events

    1. This code will refer to the following program events:

    1.1. Fundraising Any fundraising ideas or opportunities are welcomed but must be submitted to the staff at least 14-30 business days in advance of the actual event in order to have enough time to prepare for any service/aid/activity.
    1.2. Sponsored Events
    1.3. Tournaments
    1.4. Outdoor Activities
    1.5.
    H.E.A.L. Programs

    2. All members will act accordingly at all times and lead by example by showing:

    2.1. 
    Respect for any senior staff
    2.2. Respect and clear communication with your peers/teammates
    2.3. Officials and facilities to the highest degree
    2.4. Respect your peers at all times, on and off the court.

    3. The use of alcohol, tobacco and drugs are prohibited at all times throughout team events. Illegal substances have a zero-tolerance policy. While traveling members will adhere to all local laws.

    4. All participants acknowledge it will be mandatory to participate in most (if not all) outreach events unless prior notice and approval is obtained.

  • 5. Positive Behavior Policy for Events

    5.1.  Please sign in and display a level of respect when amongst peers

    5.1.1. Please notify staff or coaches of any physical or emotional issues before any activities.

    5.1.2. Members are responsible for staying hydrated and bringing their own fluids/snacks.

    5.1.3. Maintain an open line of communication with members throughout the entire program.

    5.1.4. Any physical confrontation will not be tolerated at H.E.A.L. events. Participating parties will be removed

  • 5.2. All are welcome at events if you have a good attitude and are willing to help (upon request)
    5.3. Members will sign in before any program activities or events. Failure to do so will end in the result in no participation.
    5.4. Members must alert a Staff member or volunteer immediately if they are injured in any way so that they may be treated.

  • 6. Hygiene

    6.1 Please do your best to maintain a healthy lifestyle and use the restroom in a timely fashion. We may ask to correct a matter that may be harmful to others.

    Behavioral Consequences

    Disputes/Altercations

    1. If there is an issue of any kind or someone has a difference of opinion of practice, please speak with a coach in private (outside of facilities/activity)

    1.1 Member directed at another Member: if deemed necessary; grievance forms will be filed for board members for review

    2. Members against Directors of any Committee will report to President or Board Members

    3. No one is to speak/interrupt while they are making a point. If you want to voice your opinions, raise your hand.

    4. If there is a physical or verbal confrontation/exchange, it will be reported to the Vice President/Secretary, who will create a report and file the incident. If the coach/director chooses, they may request the person/persons involved immediately exit the gym.

    5. In the event of any issues or altercations of any kind:

    5.1 A verbal warning will be issued,

    5.2 Followed by a written warning,

    5.3 Followed by a temporary suspension from events, activities, practices, and all other team events will occur. If necessary, a long-term suspension from all team related activities, with review after one year will be placed on the offender.

    5.4 If problems persist beyond temporary suspension, the board may communicate all reports and come to a decision with a majority vote on how to best handle the situation, which may lead to possible permanent suspension from H.E.A.L. Programs and Events.

  • If there has been an expulsion issued, you shall return any and all equipment (if borrowed): (Please Initial)

  • Note: When representing or wearing H.E.A.L. apparel, you represent the organization, program, and most importantly yourself. Please be polite, professional, and if asked questions you don't know the answers to about our organization or operation, refer them to Senior Staff.

    I have read and understood to the fullest of my ability and agree to comply. I understand that failure to comply with these terms can result in dismissal from H.E.A.L.
