Do you agree to be open on color, coat type, and gender within the specific litter you have chosen to place your reservation?

Do you agree to sign a legally binding SPAY/NEUTER contract through the 3rd party company Spay Secure?

Do you agree with the Visitation Policy, as stated on our website, for the safety of our puppies?

We do not schedule any work related appointments on evenings, weekends, or holidays. We typically send puppies home around 2-3pm EST on a Friday. Do you understand and agree to be this availability?

Do you agree that if for some reason, you are no longer able to care for this dog, we will be contacted about its relocation and will need to approve?

Have you ever had significant difficulties with a dog you've owned, in the past?

Is anyone, living in the home, afraid of dogs? If so, please explain.
