La Maison Pet Puppy Application

Thank you for your interest in a La Maison Labradoodle.

My puppies are priced at $3600 (plus GST)
* First payment is the holding deposit of $500 once you are on a waiting list.
* Second payment ($1500) will be due when pup is 4 weeks of age.
* Third/Final payment will be due when pup is 7 weeks of age. I will also send you paperwork to be signed.Paperwork and the final payment must be received before we send your new family member to you.

All puppies come:

de-sexed;
microchipped;
with their first of three vaccinations done;
with a written two-year health guarantee for ALA-required genetic diseases.

NOTE If you are unable to receive your puppy on the delivery date, we may have to move you to a later litter.

I will use this application form (along with any other information that you give me via texts, emails or conversations) to help me identify the Labradoodle puppy that best suits your family’s needs and desires, so please be sure to answer all questions thoroughly. Once your application has been approved, I will put you on the waiting list for a current or future litters. A non-refundable deposit of $500 is payable to reserve a puppy, which will be credited to the purchase price of your puppy.
