If you have a roster change, you will need to come back to this online roster (or click the link in your roster email) and submit the additions only. If we do not have a waiver for a player they will not be able to participate. Thank you, for your understanding.

Please take Note:
1. Provide valid parent emails. Please provide parent's email for each player. Do not simply put one email address for all players as each parent is required to complete the waiver form for their own child. Coaches will receive an email confirmation from once the parent completes their own waiver form.
2. Players may only play on one team. No mulitiple rostering is allowed.
3. Wavier confirmation will be also emailed back to the coach designated email on the roster form once it is completed by the parent.
4. If you need to add a player, simply come back to this form and resubmit this form with the addition. Do not worry about removing players from your roster.
