Completing this form serves as your official withdrawal from instrumental lessons with the tutor selected above. As outlined in the Instrumental Music Handbook and your Tutor’s Terms & Conditions, withdrawals can be completed at any time, however withdrawals must be received by the individual tutor’s deadline (2 weeks prior to the end of the current term). After this deadline, charges may apply before the withdrawal can take effect. Please note there are no reimbursements for withdrawals from the Year 5 Band Program, as enrolment is a full-year commitment.
