Terms of Service Agreement Template

1. Cleaning supplies to perform this service will be supplied by: ___________________________
2. Total costs of service will be: $_______ and payable monthly on the 1st day each month with net 15 terms. 
3. This Agreement will expire one year from the date of this agreement, but will automatically renew annually with a 3% inflation price increase if services go uninterrupted.
4. In the event this proposal proves unsatisfactory by either party. It may be terminated by a 30 day written notice by {YourCompanyName}.
5. The party receiving services may cancel the agreement only due to poor service. The receiving party MUST allow a 7 day cure period to {YouCompanyName} before the cancellation is acknowledged.
