If you no longer wish to continue organic certification, you must complete this form in its entirety to properly leave the USDA’s National Organic Program with Organic Certifiers.

By surrendering certification, this operation will no longer be able to produce and/or sell products marketed as organic under the Organic Certifier’s certificate for this farm/entity or under the scope(s) identified as surrendered above. If this operation qualifies for exempt or excluded status, the operation may sell organic product under the allowances specified under 7 CFR Part 205.101. Please note, a new application will be required if this operation seeks Organic Certification in the future.
