make sure you have read and agree to the shop's policies listed on the website on the info & faq pages.
If you do not have gmail to access the form, please send an email to moonlitfangstattoo@gmail.com with the subject heading: BOOKING FORM REQUIRED - NO GMAIL and an alternate form will be sent to you.
moonlit fangs tattoo shop is by appointment only; currently booking sessions within 2 to 4 weeks of submission (depending on availability). a response to your submission will be sent within one - five business days.
