1. Automatic Tuition draft is mandatory to be enrolled into any program at Family Sports Center and is Due the 1st of each month.

2. Foul language or any other type of profanity or bad behavior will not be tolerated.

3. Our Family Sports Center and Cheer Faces LLC family represents our town and community and is expected to conduct themselves with Class.

4. FSC members must maintain good sportsmanship to other gyms/programs around the country.

5. No food or drinks on any gym equipment including Cheer Floor.

6. You are not just joining a gym you are becoming part of our FSC and CFA family. I agree to treat any, and all gym members, athletes and staff with respect at all times.

7. All coaching will be done from the staff, please do not "coach from the stands". If you have a comment or concern, please direct it to the coach before or after practice!

8. There are NO refunds or transfer of monies of any kind. If you quit or get removed from team it will be "as is".

9. No gum allowed during practice

10. No heels of any kind are allowed on the blue floor!

11. I agree to give my undivided attention during class so I can get the best possible instruction.

12. No jewelry of any kind is allowed during practice

13. I understand that our financial obligations including, but not limited to, tuition must be paid on time. A $10 late fee will be applied on the 10th of every month.

14. I will respect all FSC and CFA property

15. No talking, or acting up during class or you will be removed from the floor.

16. You are paying for a spot in our gym. So, if you miss a class, there are no make-up classes.

17. If you break any of our rules you can be dismissed from our program without refund or transfer of monies.

Family Sports Center & Cheer Faces All-Stars LLC Program -Waiver and Release form-

As the legal guardian, or adult entrusted to care, of a gym participant(s), I recognize that potentially severe injuries, including permanent paralysis or death can occur in sports or activities involving height or motion, including but not limited to gymnastics, tumbling, trampoline, rock climbing and stunting. Being fully aware of these dangers. I consent to the aforementioned person participating in any and all Family Sports Center and Cheer Faces All-Stars LLC activities and I ACCEPT ALL RISKS associated with that participation. In consideration for allowing my child to use these facilities, I. on my own behalf and the behalf of my child and your respective heirs, administrators, executors and successors, hereby COVENANT NOT TO SUE and FOREVER RELEASE Amanda Comer DBA Cheer Faces, Family Sports Center, it's officers, directors, shareholders, employees. coaches family and/or agents. Inthe event of an accident or emergency. I would like the below mentioned child to receive the appropriate emergency medical care, including hospital care if necessary. and I hold Family Sports Center & Cheer Faces AllStars LLC. Unity All-Stars, Faith Gymnastics and its representatives harmless in their execution of this action. Additionally, 1 hereby agree to individually provide for all possible future medical expenses that may be incurred by my child as a result of any injury sustained while participating at Cheer Faces/Family Sports Center or any company within, Including. but not limited to. Unity All-Stars and Faith Gymnastics.
