MATCH SANCTION REQUEST FORM

LIMESTONE COAST COMPETITIONS

This form is to be completed by any club requesting to play a game that is outside of the scheduled competition fixtures, such as trial games.  All games must be sanctioned by Football SA.

This form is to be completed and submitted at least 5 working days prior to the requested game.  Games requested outside of this time frame, in which a match official has been requested, may not be sanctioned.
