  • Home Care Aide Plan of Care Agreements

    1. I understand that I work under the care plan created by the nurse who has assessed the patients I will be caring for.
    2. This care plan is based on patient needs, doctor's orders, and safety for the patient and the staff providing care. Only a nurse can update the care plan.
    3. I CAN ONLY PERFORM THE ITEMS LISTED ON THE CARE PLAN.
    4. If a patient/family asks me to perform a service, not on the care plan, will say I legally can not.
    5. I (or the patient/family) can call the office and ask for a change in the plan.
    6. If I provide ANY service, not on the care plan, I am solely responsible for whatever happens as a result of my actions.
    7. Meet and Greet is not a visit and does not fall under the care plan. I cannot perform any duties during my meet and greet time.

    I CANNOT DO ANYTHING THAT is NOT ON THE PLAN OF CARE. IF I DID AND A PROBLEM HAPPENED, THE

    AGENCY MAY NOT SUPPORT ME AND I ALONE MAY BE RESPONSIBLE.
