Informed Consent Form

All those signing up to use programming from the BMB on Demand app must complete this form before beginning training.
Please read through each and every item in this informed consent and mark each checkbox provided as an affirmation to the understanding of the provisions below.

By subscribing and using the programming provided on the BMB on Demand app, you are accepting that your safety is your responsibility. You must assess risks and make safe choices when exercising. Use proper form and best judgement.

If you disagree to any terms in this contract, you do not have permission to use BMB on Demand.

Purpose*

I understand exercises I perform in this program may include but are not limited to, aerobic activities, walking, running, cycling, rowing, and weight lifting with the use of machines and free weights. I am responsible for using good form on all forms of exercise.
Risks*

I understand that during the conduct of exercise, the reaction of my heart, lungs, and blood vessels, etc., cannot be predicted with accuracy, thus risks may arise due to adverse changes that may lead to heart attack, high blood pressure, or stroke.I understand that during training, injuries may arise such as muscle, ligaments, joints and tendons; and in I have been given proper information in order to prevent or minimize these occurrences.I understand that it's my responsibility to get a yearly physical exam from my doctor to be cleared for exercise.
Waiver*

I understand that I shall solely be responsible, and BMB shall not be held liable for any damages and injuries arising from my fitness training.I understand that during training, injuries may arise such as muscle, ligaments, joints and tendons; and in I have been given proper information in order to prevent or minimize these occurrences. If I am unsure, it's my responsibility to communicate with my coach to monitor my form.I hereby release and fully and forever discharge Big Mountain Barbell, its coaches and agents from any and all claims, demands, and/or damages therein.It understand that it is solely my responsibility to monitor my body and its’ reactions and determine if it is appropriate to use the sauna. I am responsible for my safety and well-being.
Confidentiality*

I am aware that all information obtained by Big Mountain Barbell from me, such as fitness records, personal information, among others, shall be treated as confidential information and that the fitness center holds these information as privileged information. These information shall not be disclosed nor shared to anyone without my prior written consent.
By signing this form, I have had the opportunity to ask questions and those were answered to my satisfaction. I understand all the terms, my rights, as well as my obligations herein. I voluntarily give my consent and I declare that I was not induced or coerced into signing this form.
