- On Occasion, Joyous Education Group may receive request from other professionals or community to observe in the class. We attempt to do this with the least disturbance to the child's routine. I hereby to give permission for my child to be present on these occasions.
- I'm authorising permission (Photo and video)for my child's photo/video to be used in the following purpose
- I hereby to give permission for my child to go on field trips (in accordance to Joyous's thematic curriculum) arranged by team of Joyous. I understand that my child will be under direct supervision and care of staff in charge. I will be informed in advance of any special field trips.
- Acknowledgement of Incident SOP
  
  I hereby acknowledge that i have read and understand the Incident SOP in JOYOUS.
  
  I voluntarily sign it and hereby give permission to JOYOUS for emergency transportation and/or treatment in the event of illness or injury. I further certify that my child is in good physical condition and has no medical or physical conditions that would restrict his/her participation in programs/classes/therapy sessions conducted. I understand that JOYOUS has the right to deny admittance to any student not meeting the standards of the program as it sees fit.
