 ABOUT YOUR APPT:

• The test typically takes 5-10 Min
• Your test will be performed on the shoulder. Please wear clothing that accommodates this. The test is quick and painless.
• It is recommended to drink a glass or bottle of water 30 min to an hour before your appointment.
• You're welcome to bring a guest or kiddos. Please note ALL MALES must stay in the lobby while the test is being performed to prevent contamination.
  
• IF bringing a MALE toddler or baby alone, please bring a stroller for them to sit in while the test is performed (takes 5 minutes)
• If adding a 2D scan, please note we offer this service from 7 weeks, as we do not perform vaginal ultrasounds, and the baby is often too small for us to see clearly before 7 weeks. RESCANS are not offered if the baby can't be seen or heard -due to early pregnancy.
• Your sample will be overnighted using FedEx, and samples are logged in and will be confirmed received the following day (MON-FRI)
• Samples done on Saturday will be sent on the following MONDAY and results for NEXT-DAY will be announced on TUES and 2 Day turn around will be announced WED.
• Results will be sent directly to your email or the person of your choice.
  Results are typically received by 5:30 pm, but at late as 8 pm (for tests performed MON-FRI)
  Tests done on FRI will get NEXT-DAY results on SAT. No test results are provided on SUN or HOLIDAYS.

SNEAK PEEK IS AVAILABLE FROM 6 WEEKS PREGNANT.
By completing this form, I confirm that I’m at least 6 weeks pregnant. I understand that testing before can cause an inconclusive or inaccurate test result. I understand Sneak Peek (Gateway Genomics LLC) & Luxe Baby Ultrasound are not responsible for test results done before 6 weeks. A re-test will not be offered at no cost for clients if the test is done before 6 weeks. Refunds will not be offered or available. Please make sure to confirm your pregnancy and how many weeks you are pregnant with your medical care provider.

FEDEX SHIPPING NOTICE

I understand that FedEx will be handling the delivery of my sample to the lab. I also understand that Luxe Baby is NOT responsible if there is a loss of the sample, damage to the blood sample (which leads to inconclusive test results), or delay in shipping due to FedEx and their handling of my sample as Luxe Baby has no control over this. In the event there is damage or loss to a sample we will offer a retest at no charge. Also if there is a delay in shipping all Luxe Baby can offer is a refund for your $20 overnight fee. In the event of an inconclusive test result or incorrect test result shipping fee will not be refunded.

PLEASE ALSO NOTE

I also understand that while LUXE BABY offers this test as a service, it does not own Sneak Peek or Gateway Genomics LLC, In the event that the test comes back with an inaccurate determination or the test comes back with inconclusive results, LUXE BABY isn't to be held responsible. NOTE: Refunds for inconclusive tests will be provided after a second test is performed IF a second inconclusive result is given. REFUNDS for tests that are later found to be inaccurate will be provided after a declaration of birth- with the stated gender, provided by the hospital or professional attending the birth. Refunds for inaccurate tests are handled by SNEAK PEEK.
Please also note that while pregnancy loss is terrible we can not offer refunds for sneak peek tests in the event of loss after the test is performed and delivered. We are so sorry.

Please also understand credit card chargebacks are not possible for inaccurate or inconclusive tests results after accepting the above terms. 

CONSENT TO DNA TESTING AND TERMS OF SERVICE

By signing below, I consent to this testing. I understand that all materials and products provided by Gateway Genomics, LLC (“Gateway”) are provided for informational purposes only and that I should consult with a physician or healthcare provider regarding any questions pertaining to fertility or pregnancy. I further recognize that this does not replace a woman’s need to visit a doctor during pregnancy or to have an ultrasound or other pregnancy-related tests performed. I understand that Gateway is not liable for any damages caused to me or anyone else as a result of using this product. Furthermore, I assure that I am not using this product for reasons relating to gender selection or any other medical purpose. By making a purchase from Gateway, I agree to release, indemnify, and hold harmless the company, all of its officers and employees from all liabilities associated with the product(s) purchased. I understand that Gateway is not liable for any actions that might be taken by myself or anyone else as a result of using this service. I agree that any disputes arising between me and Gateway shall be resolved by final and binding arbitration under the rules and auspices of the American Arbitration Association, to be held in San Diego, California, in English. You and Gateway agree that each may bring claims against the other only in your or its individual capacity, and not as a plaintiff or class member in any purported class, consolidated, or representative action.
