27th Luncheon Table Sponsorship Form

Thank you for your support of child abuse prevention services in NE Florida! The 27th Annual Child Abuse Prevention Luncheon will be held:

Tables of 8 are offered at the following sponsorship levels (Please check one):

Community Partner Level, $1000. You will receive a table for 8, table signage and recognition in our Event Program.Major Supporter Level, $2,500. You will receive a table for 8, table signage, enhanced recognition in our Event Program and acknowledgement during the program.Champion Level, $5,000. At this level, you will receive two tables for 8, table signage, top billing/recognition in our Event Program and acknowledgement during the program.

The event will be held on Thursday, April 20th, and draw approximately 250-300 attendees. This event is the only opportunity for Social Work professionals to get the recognition they so deserve for the work they do throughout the year, working with the children and families of NE Florida.

The Exchange Club Family Center is a 501 (c) 3 nonprofit organization, and all sponsorship contributions are tax deductible, and all proceeds go the support our Parent Aide program. The Parent Aide program is an in-home visiting program that provides valuable information and parent education to families struggling with a wide variety of issues within our community.
