This is your Summary Plan Description. The wait period for elegibility is 1st of the month.

This summary plan description will provide a general overview of the IRC Section 125 Flexible Benefits Plan, which is a benefit program that allows you to choose among specific benefits that are not subject to Social Security Tax, Federal Income Tax, or State Income Tax.

This summary plan description should be carefully reviewed so that you can understand the porovisions of the plan.

There is a plan document on file with the employer that provides detailed informatiuon on the operation of the plan.

If there is a conflict between the summary plan description and the plan document, the summary plan description will be disregarded and the acutal plan provisions will be controlling. I understand that once I am a participant, I cannot change my deduction amounts during the Plan Year unless a qualifying status change occurs.
