Santa Fe Youth Symphony Association values and highly encourages every student to participate in their school's corresponding music program. This form is to only be completed when a student/family feels that they need a reasonable exception to our co-enrollment policy. Once completed, this application will be reviewed by an SFYSA director and the school district music director or the music department head, and will be approved or denied.

We believe that supporting a co-enrollment policy is beneficial for our local school music programs. To create a strong application for an exception to this policy, we strongly encourage you to select one or more options to show that you will continue to support your school music program in another capacity and upload any supporting documentation.
