Fill out the form carefully for registration. Only ONE form is necessary for siblings enrolling in the same course. Having trouble registering? Email register@engagearts.com! IF your student was enrolled in the same class the previous month/session, you do NOT need to fill out this form. Please just see the email sent on the 15th of every month.

I understand that Engage will contact me if any ceramics projects need to be picked up at a later date. I understand that if I do not pick-up projects within 3 months of Engage contacting me, my/my child's left-behind project(s) will be donated.
Confirmation of Terms and Conditions*

I have read and understand the terms and conditions regarding my (and/or my child's) enrollment at Engage Art Studio. I understand that if I do not want pictures of my child to be uploaded to Engage Art Studio's social media, I must notify via email to studio@engagearts.com.
Refund/Cancelation/Pro-Ration Policy*
