    This form is for applicants interested in being transporters for Animals Lives Matter of East Texas. Transporters are volunteers who will use their own vehicles to transport animals to fosters, rescues, vet, etc. within a 100 mile radius of Livingston, Texas. This form does not guarantee an approval and Animals Lives Matter of East Texas reserves the right to cancel an applicant or transporter at any time deemed fit. All gas expenses will be reimbursed by submitting receipts via email to almofeasttexas@gmail.com.

  • By signing this application form, I accept the following requirements of the agreement:

    1. The information about driver's license is valid.

    2. My vehicle is fully insured.

    3. I accept all the responsibilities for injury, damage, and traffic violations.

    4. I acknowledge that Animals Lives Matter of East Texas is not responsible for any wear/tear, soils, or damages caused by animal being transported.

    Liability Release Waiver

    I recognize that while performing my services in a voluntary capacity, in handling animals there may be a risk of injury, and I agree to assume that risk. On behalf of myself, my heirs, and personal representatives, I hereby release and hold harmless Animals Lives Matter from any and all claims, causes of action, or demands of any nature connected to my volunteer service.

    I also agree to release and hold harmless Animals Lives Matter for any and all damages to my personal property while performing my volunteer services.

    I acknowledge that I have read and fully understand the terms and conditions of the foregoing liability release waiver.
