This form is required to be completed in order to release any bond or letter of credit that was used for this project. Even if you piggy backed off of a major utility company, you must fill and submit this form.

This date should be the same or a few days off from the one provided on your application.
ALL restoration of jobs must be "Inspected, Approved and Released" to be considered complete by City standards in order to have any active bond funds, piggy backed bond funds, letter of credit funds or funds paid out and put into escrow account.

All restoration work must be documented digitally via uploaded pictures.

In addition to the digital pictures, an in person inspection may be warranted based upon the scope of the original permitted work. This is at the discretion of the Public Works Director or his desginee and will be scheduled at the earliest convenience of all parties.

Delay in this inspection will cause delay or denial of any future applications.
Please provide pictures of your completed work.*
