This form must be completed and handed to your line manager whenever you wish to take time off, whether it is for one day or longer. As much notice as possible must be given before a holiday request will be authorised. It would be helpful if a minimum of four weeks notice is given, unless in exceptional circumstances, as agreed with the Care Manager. Caremark will notify you within 7-10 working days as to whether your holiday has been authorised. Please refer to the relevant sections in your Care Worker Services Agreement and /or Contract of Employment regarding holidays. PLEASE NOTE - the holiday year starts from 1st January and ends 31st December.
