Participant Committment

I understand by completing this form my child(ren) will be participating in the 2023 ADC Student Show at Orange Coast College on June 10, 2023. I further understand this annual showcase includes a Tech Rehearsal, Photo Day and stage performance at the Show which are Mandatory events if agreeing to participate.
Costumes:

I understand I will be responsible to measure my child for her/his costume, selecting the size and placing the order online (specific costume details will be provided to parents, see Avanti's website for costume instructions).

PARTICIPATION FEE is $95.00 for one student up to three classes, $10 each additional class participating. Siblings are $45 each for up to three classes, $10 each additional class participating. Participation fee includes media download of entire Show performance and commemorative t-shirt for each dancer.

CONSENT TO CHARGE CREDIT CARD | As the parent or legal guardian of the above-named participant, I hereby give my consent to charge my credit card currently on file in my parent portal (Jackrabbit) I understand this is a minimum fee of $95.00 (and may be more depending on the total classes performing) which is non-refundable once charged. If student is later unable to participate, parent/guardian acknowledges the refund policy for participation fees paid below:

Cancellation requests received in writing to info@avantidancecompany.com by 2/15 will receive a 100% refund less $25.00 administrative fee. No costume fees paid will be refunded.

Cancellation requests received in writing to info@avantidancecompany.com by 3/15 will receive a 50% refund less $25.00 administration fee. No costume fees paid will be refunded.

Cancellation requests received 3/16 or later will not receive a refund. Only those with a medical emergency will be eligible for a 50% refund with a physician’s note in the event of severe illness or injury preventing participation. No costume fees paid will be refunded.
