You have requested a written prescription for patient listed above to purchase a medication or prescription diet from an Online Pharmacy.
As always, the doctors and staff of Urban Veterinary Associates are committed to the health and safety of your pet. Before we provide you that prescription, we request this waiver be signed for a Release of Liability.
By signing this document, you acknowledge that we have informed you of the following:
- Purchasing prescription medications or food from an internet pharmacy may be harmful to your pet. The National Association of Boards of Pharmacy advises all consumers to be aware that many Online Pharmacies sell counterfeit, contaminated, unsafe and short-dated drugs, both for humans and animals. These drugs are known to be shipped and stored in an unregulated manner.
- Many of these sites are operating illegally, from foreign countries where medications/diets are shipped without oversight or proper handling. Even the best of these pharmacies obtains name brand drugs/diets from unethical sources.
- Manufacturers of medications and food will not warrant safety, purity, or efficacy when marketed through these sources. However, if purchased through a licensed Veterinarian, all warranties are honored. Including refunds for uneaten food, even if the bag is opened.
- Our clinic cannot take responsibility for the safety of medication or foods purchased through an Online Pharmacy, nor can we track the continuity of administration when filled outside of our hospital.
- Rebates offered by the manufacturer are not honored or available through Online Pharmacies
- Often our clinic prices are more competitive than Online Pharmacies. Sales from our pharmacy help to maintain affordability of the services and staff you have come to value.
By your signature below, you agree to hold Urban Veterinary Associates harmless in this matter. Should you have any questions concerning this matter, feel free to contact our office at (630)-541-8088 for further information. Once we receive this signed waiver, a nurse will give you a call to pick-up a written script for the appropriate medication or diet the patient listed above. There will be a prescription management fee per written script. This waiver is valid for 365 days of the date signed.
