This is a form to express interest for you/your student to become a youth brand ambassador (YBA). This interest form does not commit you/your student to Game-On's program.

If you/your student decides to be part of this program, you will be required to complete a membership packet, purchase a 1 month membership ($25), and sign off on YBA agreements.

Consecutive months of membership will be awarded for participation, which includes membership perks. Earning potential for referrals, starts immediately.
