Please read and initial. You are voluntarily participating in the workouts associated with Boxing Blitz with Anthony Fiore at Gold's Gym Middletown. You assume the risks of injury from exercises performed during the boxing workouts. You understand that you are engaging in physical exercise, which includes the use of boxing gear and equipment. You agree to waive any claims or rights you might have to sue the health club (Gold's Gym), its owners, officers, employees (including Anthony Fiore), or agents for injury to you as a result of these boxing exercises and activities. You are hereby advised that you should be sufficiently physically fit for exercise activities and should have consulted a physician prior to undertaking a physical exercise program.
